A. Education/Experience

1. High school diploma or equivalency
2. Some experience in mental health related field
3. Equivalent combination of education and experience meeting
Office of Mental Health standards
4. Valid New York State or Pennsylvania driver’s license

3. Ability to work with team approach
4. Good problem solving skills
5. Ability to accommodate flexible work schedule
6. Ability to manage time
7. Ability to lift over 40 pounds and be physically able to perform all required duties
8. Familiar with computer programs (e.g. Work, Excel)

A. Primary Duties

1. Writing or assisting in writing reports and documenting individual progress
2. Participate in team planning meetings and case reviews
3. Teach/assist in developing/maintaining ADL skills
4. Act as a role model
5. Perform routine site household maintenance and upkeep
6. Assist in teaching self-medication skills, utilization of health services and proper nutrition
7. Assist individuals in identifying personal recreational interests
8. Identify and use community based leisure/recreation activities
9. Provide appropriate restorative services to meet individual needs
10. Maintain case records and provide documentation according to program standards
11. Intervene appropriately to assist in crisis management
12. Understand and follow all program policies and procedures
13. Report all household or resident sensitive situations to supervisor
14. Participate in and successfully complete all required staff training and development activities
15. Participate in intake and discharge planning
16. Assist individuals in the development of decision-making skills
17. Communicate with other service providers
18. Provide transportation as needed
19. Assist individuals in accessing community services
20. If assigned to a CR then you will assist with the preparation and share meal times with residents
21. Communicate with supervisor and senior counselors on all pertinent issues
22. Represent the Agency and program in the community and with other service providers
23. Establish relationship with all residents and families as appropriate
24. Regular and substantial contact with individual receiving services
25. Perform other related tasks as directed by supervisor
