Outreach and Recovery Coordinator

Hours: Full-Time Monday-Friday 8:30am-4:30pm

QUALIFICATIONS

Bachelor’s degree in health/human service field; or  NYS teacher’s certificate and a bachelor’s degree; or RN and a Bachelor’s degree  AND 2 years of experience in providing direct services to people with behavioral health diagnoses

OR

Bachelor’s degree in any field with 5 years of experience working directly with persons with behavioral health diagnoses or a CASAC

OR

Master’s degree in health/human services field and one year of experience working directly with persons with behavioral health diagnoses

Ability to legally operate a vehicle in NY State

Good organizational and time management skills

Good oral/written communication and interpersonal skills

Knowledge of community agencies and resources

Familiar with computer programs (e.g. Word, Excel)

The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighting up to 40lbs.

Provide outreach to engage vulnerable populations in service assessments

Conduct initial and on-going assessments

Develop plans of care based on the assessments of service needs

After clients have been assessed and plans of care have been developed clients are referred to appropriate providers.  No ongoing care management is provided by this position.

Complete required reports/notes in a timely and professional manner

Maintain documentation necessary to substantiate billing/funding

Understand and follow all program policies and procedures

Must utilize software technology pertinent to program

Act as a linkage to community services including medical, behavioral, residential, benefit services, promoting health and wellness activities

Participate in team meetings

Ensure that a person centered planning process is employed in delivery of services

Regular and substantial contact with individuals receiving services

Participate in and successfully complete all required staff training and development activities

Represent the agency and program in the community and with other service providers

Perform other job related activities as directed by supervisor
