ACT Vocational Specialist Behavioral Health Professional Level 1

Salary $22.05-$24.82/hr.                                          FLSA Non-Exempt

 I.                Qualifications:

A.    Education / Experience

1.      Bachelor’s degree in Human Services or related field required

2.     Minimum two years’ experience in behavioral health

3.     One year experience in vocational rehabilitation or vocational experience with disabled individuals preferred.

 B.    Skills

 1.     Knowledge of psychosocial rehabilitative services

2.     Knowledge of models of supported employment/Individual Placement and support (IPS)

3.     Ability to complete a vocational assessment to determine mental health needs in the workplace or educational setting.  

4.     Ability to coordinate and participate in the development of

individualized and comprehensive service plans.

5.     Ability to work within a multi-disciplinary team using a psychiatric rehabilitation model.

6.     Ability to effectively communicate both orally and in writing.

7.     Excellent organizational and time management skills

 II.             Responsibile to:   ACT Team Leader

 III.           Major Functions:

 A.    Program

 1.     Participates with the ACT Team and enrolled individuals in the development of a vocational assessment, comprehensive assessment and service planning. 

2.     Networks in the community to locate and create volunteer and paid work opportunities for individuals enrolled in ACT services. 

3.     Coordinates vocational activities and works closely with IPS specialist to provide medically necessary interventions related to employment needs of the individual.

4.     Participates as a team member in the frequent monitoring, assessment and evaluation of the mental health status for individuals; including significant others, the home and the community in which the individuals lives.

5.     Monitors and coordinates the delivery of necessary services including transportation, making appointments, assistance with housing, law enforcement, social services, employers and doctors.

6.     Develops a therapeutic relationship with individuals and assists them with problem solving, crisis management and the development of social supports.

7.     Provides service coordination/chart management for 10 individuals.

8.     Maintains accurate and up to date records for individuals enrolled in ACT.

9.     Attends daily team meetings and provides regular updates on status of individuals.

10.  Rotates assignment of weekly and weekend after hour’s coverage.

11.  Provides technical assistance and education to members of the team in the area of education and employment.  Develops materials and strategies for career interventions in conjunction with the team.

B.    Catholic Charities / Community Related

1.       Adheres to Catholic Charities, NYS Office of Mental Health, ACT Institute and Broome County policies and procedures.

2.       Maintains awareness of community resources relevant to individual needs. 

3.       Attends relevant trainings and professional meetings and represents agency business in professional manner.

4.       Possession of a valid driver’s license.
