Care Manager - Level 1 – Protective Services for Adults

FLSA  (Non-Exempt)

I.                QUALIFICATIONS:


1.     Associates’ degree in health or human service field preferred

2.     High School Diploma or GED required

3.     One year experience working with special needs population


1.         Good oral/written communication skills

2.         Good interpersonal skills

3.        Knowledge of area human service agencies

4.        Ability to work with service providers

5.        Familiar with computer programs (e.g. Word, Excel)

6.        The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing 40 lbs.

7.        Ability to legally operate a vehicle in NY State

II.        RESPONSIBLE TO: Supervisor


1.               Maintain required documentation

2.               Ensure that a person centered planning process is employed in delivery of services

3.               Link and connect to natural supports and community resources

4.               Provide crisis intervention as necessary

5.               Complete required reports/notes in a timely and professional manner

6.               Comply with all applicable regulations

7.               Ensure that individual’s rights are protected

8.               Advocate for individuals

9.               Maintain documentation necessary to substantiate billing/funding

10.            Participate in and successfully complete all required staff training and development activities

11.            Represent the agency and program in the community and with other service providers

12.            Regular and substantial contact with individuals receiving services

13.            Perform other job related activities as directed by supervisor

14.            Understand and follow all program policies and procedures
