GRADE  8          FLSA Non-Exempt


A.              Education/Experience

1.               NYS License Practical Nurse

2.               One year experience with DD/MH population preferred

3.               Two years nursing experience preferred

B.              Skills

1.               Good observation skills

2.               Good interpersonal skills

3.               Ability to work with people at different functioning levels

4.               Ability to work with the Interdisciplinary Treatment Team

5.               Ability to support staff in health care issues

6.               Familiar with computer programs (e.g. Word, Excel, EHR)

7.               Familiar with community agencies and resources

8.               Ability to legally operate a vehicle in NYS

9.               The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing up to 40 lbs.

II.        RESPONSIBLE TO: Supervising Nurse/Care Manager


A.              Primary Duties

1.               Provide medication education to staff and individuals

2.               Work as part of a Care Coordination team

3.               Assist/cover for other nursing staffs absences

4.               Complete required documentation

5.               Oversee/review medication ordering process

6.               Write or assist in writing reports and documenting individual progress

7.               Identify and advocate for individual rights

8.               Teach/Assist in developing/maintaining ADL skills

9.               Administer/supervise medications and treatments as per doctor’s orders

10.            Assist in teaching self-medication skills, utilization of health services and proper nutrition 

11.            Coordinate and conduct care planning activities for a designated case load of recipients

12.            May utilize health information technology to coordinate care and communicate with providers and deliver services

13.            Act as medical resource consultant for team members

14.            Promote health and wellness activities for recipients via individual or group sessions

15.            Provide crisis intervention services as necessary, may be required to participate in after hours on call rotation

16.            Maintain certification in required areas

17.            Attend specialized medical appointments deemed necessary

18.            Report all sensitive situations to supervisor

19.            Provide blood borne pathogens and other training to program and agency staff as needed

20.            Understand and follow all program policies and procedures

21.            Represent the agency and program in the community and with other service providers

22.            Participate in and successfully complete all required staff training and development activities

23.            Regular and substantial contact with individuals receiving services

24.            Perform other related tasks as directed by supervisor
