Teen Transitional Living Program (TTLP)

YOUTH ADVOCATE

I.                QUALIFICATIONS

A.              Education:

High school diploma or a GED

B.              Experience:

Some relevant experience working with youth

C.              Skills:

1.               Good oral and written communication skills

2.               Good judgment and decision making skills

3.               Good interpersonal relationship skills

4.               Ability to drive an agency vehicle

5.               Must be able to lift up to 40 lbs.

6.               Computer skills adequate to job duties

II.             RESPONSIBLE TO:   TTLP Supervisor

III.           MAJOR FUNCTIONS

1.               Provides additional support/supervision to TTLP residents and visit apartments to ensure residents safety

2.               Provides recreational activities to residents according to their service plan

3.               Provides residents with individual skill building and IL as it pertains to their service plan

4.               Follows up with daily, weekly schedule set by Case Managers

5.               Documents case records, daily and monthly record reports

6.               Informs Case Managers of resident progress, problems and/or concerns

7.               Maintain a flexible work schedule based on client/program needs

8.               Intervenes to resolve crisis situations

9.               Provides transportation as needed

10.            Participate in staff meetings, agency trainings, on-call
