Catholic Charities to hold "Health Home" Fair

Catholic Charities will host a Health Fair on Thursday, July 18 from 11 AM until 2 PM at 290 Front Street, Binghamton to encourage participation in their Health Home Program. All Catholic Charities current and potential Health Home members are invited to attend.

‘Health Home’ is a New York State program designed to provide comprehensive care management services for Medicaid beneficiaries through an integrated network of health and social service providers.

Catholic Charities of Broome County has been chosen as a Lead Agency under the New York State plan. The Health Home program targets Medicaid recipients with chronic health conditions in an effort to better serve their individual needs, while reducing Medicaid costs.

Health and social service providers will be available to provide information about their services. Participants include Cornell Cooperative Extension, American Heart Association and the Southern Tier Independence Center. The fair will feature healthy picnic food, raffles and the Ross Park Zoo Mobile.
