Behavioral Health Professional – Level 2 – Substance Use Specialist
Assertive Community Treatment
$24.25-$27.18 per hour FLSA Non-Exempt
I. QUALIFICATIONS:
Education/Experience
1. Master’s degree in a Human Service field required
2. Knowledge of treatment options for individuals with Dual Diagnosis
3. Minimum of three years’ experience working in Behavioral Health
Skills
1. Knowledge of mental illness diagnosis
2. Knowledge of Dual Disorders
3. Ability to work within a multidisciplinary team
4. Good organizational and time management skills
5. Good oral/written communication skills
6. Good interpersonal skills
7. Knowledge of community agencies and resources
8. Ability to work with service providers
9. Familiar with computer programs (e.g. Word, Excel)
10. The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing 40 lbs.
11. Ability to legally operate a vehicle in NY State
II. RESPONSIBLE TO: Supervisor
III. MAJOR FUNCTIONS:
1. Monitor and assess individual Mental Health status
2. Identify individual needs based on ongoing assessments
3. Develop a therapeutic relationship with individual and assisting with problem solving and crisis management
4. Educate individual and/or family on diagnosis/symptom recognition
5. Conduct care plan meetings with all relevant service providers
6. Act as a linkage to community services including medical, behavioral, residential, services, etc.
7. Utilize software technology pertinent to program
8. Maintain required documentation and deadlines
9. Ensure that a person centered planning process is utilized in delivery of services
10. Link and connect to natural supports and community resources
11. Provide crisis intervention
12. Ensure that individual rights are protected and advocate for individual
13. Participate in and successfully complete all required training and development
14. Represent the agency and program in the community and with other service providers
15. Regular and substantial contact with individuals receiving services
16. Actively participate in team meetings
17. Perform other job related activities as directed by supervisor
18. Understand and follow all program policies and procedures
