Care Manager Level 2
Salary $19.95-$22.45/hr. FLSA Non-Exempt
I. QUALIFICATIONS
A. Education
1. Bachelor’s degree in the field of human services related field is required.
B. Experience
1. Minimum of one year experience working in providing direct social work service to special needs individuals including those living with a mental illness, developmental disability, substance use or elderly.
2. Knowledge of community mental health services, services available to developmentally disabilities and geriatric.
C. Skills
1. Excellent communication skills, both oral and written.
2. High degree of organizational skills to meet assigned work demands.
3. Demonstrated ability to work with a team as well as function independently.
4. Good computer skills with working knowledge of Microsoft Office necessary.
6. Knowledge of special needs individuals including, mental illness/psychiatric disorders/developmentally disabled and geriatrics.
7. The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing
40 lbs. or less.
8. A valid driver’s license and the ability to legally operate a vehicle in NYS.
9. Regular and substantial contact with those individuals receiving services.
II. RESPONSIBLE TO: Protective Services for Adults Team Leader
III. Major Functions
A. Consumer/Program Related
1. Assist individuals in identifying and achieving goals and developing written service plan.
2. Assist individuals in making informed choices based on needs and interests.
3. Provides case work counseling to motivate the individual or family to increase their own confidence and capacity to handle problems.
4. Coordinates service plans with family and providers and ensures quality of care.
5. Conducts home or community-based visits.
6. Provides transportation, as needed.
7. May appear in court as an advocate or witness.
8. Facilitate service delivery, including making and assistance with keeping appointments.
9. Assist with locating and creating support networks in the community.
10. Assist recipients in learning to use financial resources responsibly, including assistance with budgets.
11. Provide crisis intervention services as necessary.
12. Promote self-advocacy and personal empowerment.
13. Maintain case records, progress notes and assessments, as required in compliance with Agency, County and Office of Children and Family Services regulations.
14. May carry caseload of up to 40 recipients and provide a minimum of one visit per month.
15. Complete and submit monthly reports related to services provided and individuals served.
16. Encourage medication, education and compliance.
17. Communicate regularly with other providers and individuals to develop or enhance consistency of services.
18. Coordinates Representative Payee services as needed.
19. Participate in ongoing training, attend meetings as required.
20. Physical condition commensurate with the demands of the position.
21. Perform other related duties as required.
