Childrens Community Service Provider Level 1 - Per Diem

TITLE:                      Children’s Community Service Provider Level 1

DIVISION:                Youth Family and Community Service

DEPARTMENT:        CFTSS-PSR

HOURS:                   Per Diem

GRADE:                   FLSA Non-Exempt - $20/hr.

I.                   QUALIFICATIONS:

Education/Experience

1.     Associates’ degree in health or human service field preferred

OR

2.              High School Diploma or GED required and

3.     Three years’ experience working with special needs population

Skills

1.            Good organizational and time management skills

2.            Good oral/written communication skills

3.            Good interpersonal skills

4.        Knowledge of area human service agencies

5.        Ability to work with service providers

6.        Familiar with computer programs (e.g. Word, Excel)

7.                    The ability to climb stairs, bend, stoop, sit, stand, reach and lift items     weighing 40 lbs.

8.        Ability to legally operate a vehicle in NY State

II.        RESPONSIBLE TO:            Supervisor

III.       MAJOR FUNCTIONS:

1.                  Provide direct service to children in their home, school or community

2.                  Provide direct services to children one-to-one and/or in groups. Delivery of services will follow a plan of care developed by the client and family

3.                  Ensure a person centered planning process in delivery of services

4.                  Provide appropriate care and supervision of all children while on outings

5.                  Provide crisis intervention as necessary

6.                  Maintain required documentation and complete in a timely manner to substantiate Medicaid billing

7.                  Attend supervision and staff meetings as necessary

8.                  Participate in multidisciplinary team meetings for quality care of individuals and represent the agency in a professional manner

9.                  Comply with all applicable regulations

10.              Advocate for and ensure the individual’s rights are protected

11.              Provide face to face service to children at least 50% of working hours

12.              Participate in and successfully complete all required staff training and development activities

13.              Perform other job related activities as directed by supervisor

14.              Understand and follow all Agency and Program policies and procedures
