Salary $19.95-$22.45/hr. FLSA Non-Exempt
I. QUALIFICATIONS:
Education/Experience
1. High school diploma or equivalency
2. 2 years’ experience in human service field
1. Effective written and observational skills
3. Ability to work in a team approach
4. Ability to work with people at different functioning levels
5. Good problem-solving skills
6. Ability to accommodate flexible work schedule
7. Familiar with computer programs (e.g. Word, Excel)
8. Ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing up to
40 lbs.
9. Ability to legally operate a vehicle in New York State
II. RESPONSIBLE TO: Supervisor
III. MAJOR FUNCTIONS:
Shift Lead:
1. Assist Supervisor and/or Recreation Therapist to organize activities, tasks and assign duties to be completed during shift and delegate to staff according to ability and time
2. Ensure daily schedule and planned activities are followed
3. Make decisions on unanticipated occurrences during shift.
4. Conduct monthly Fire Drills with residents and ensure documentation is completed.
5. Complete monthly house maintenance checks, fire extinguisher checks, and weekly CO2 Detector checks and ensure documentation is completed.
6. Ensure vehicle logs are filled out accurately.
7. Ensure that receipts from any purchases made during shift are signed and logged in the binder.
8. Ensure vehicle keys and med room keys are turned in at the end of shift.
9. May be assigned to participate in rotating afterhours/weekend on call coverage.
DSP responsibilities:
10. Act as a role model while always providing supervision to residents
11. Teach/assist in developing/completing ADL skills
12. Perform routine household maintenance and upkeep
13. Assist with planning/preparing meals and clean-up as needed
14. Supervise resident’s self-administration of medications and/or treatments as per doctor’s orders and document in log if taken as prescribed
15. Provide transportation as needed
16. Assist in crisis management- crisis avoidance, de-escalation and intervention and follow resident’s behavioral support plan
17. Participate in scheduled staff meetings, team planning meetings and case reviews
18. Complete progress notes and documentation in daily log as required
19. Assist resident’s in working towards achievement of service plan goals & objectives
20. Understand and follow all program policies and procedures and OMH regulations
21. Report all household or resident sensitive situations to supervisor
22. Participate in and successfully complete all required staff training and development activities
23. Participate in intake and discharge planning
24. Participate in family meetings as requested
25. Represent the Agency and program in the community and with other service providers
26. Perform other related tasks as directed by supervisor
