Supervisor – Level 2 – Adult Care Management
GRADE 9+ FLSA (Non-Exempt) - $22/$24.76/hr.
I. QUALIFICATIONS:
Education/Experience
1. Bachelor’s degree in Human Services field preferred
2. Associates degree in Human Services field required
3. Two years’ experience working in human service field
4. Some supervisory experience required
Skills
1. Good problem solving skills
2. Ability to communicate effectively
3. Good leadership and team-building skills
4. Ability to assign and delegate tasks
5. Familiar with computer programs (e.g. Word, Excel)
6. The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing 40 lbs.
7. Ability to legally operate a vehicle in NY State
II. RESPONSIBLE TO: Manager
III. MAJOR FUNCTIONS:
1. Understand and implement all agency and program policies and procedures
2. Maintain compliance with program quality improvement plan
3. Submit required reports and statistical information in a thorough and timely manner
4. Monitor, evaluate and provide direction to staff
5. Assure that all staff training requirements are met
6. Assign staff and maintain adequate staff coverage to meet client/program needs
7. Convene periodic staff meetings and submit minutes to Manager
8. Keep Program Manager informed of all pertinent program information
9. Assure plans are written and implemented according to program procedure and incorporates person centered elements
10. Monitor that incidents are properly reported and documented per regulation
11. Understand and monitor client eligibility for benefits and entitlements
12. Assure that staff appropriately record all individual contacts and interventions according to program standards
13. Assure that all individuals receiving services are provided instruction, assistance, interventions, supervision and supports necessary to maintain competence in basic skills and behavior
14. Assure referrals are processed in a timely manner
15. Coordinate and document admission and discharges
16. Monitor expenditures and program accounts
17. Maintain inventory of furnishings and program supplies
18. Represent agency and program in the community and with other service providers
19. Establish and maintain relationships with individuals receiving services
20. Monitor and advocate for individual rights as needed
21. Regular and substantial contact with individuals receiving services
22. Participate in and successfully complete all required staff training and development activities
23. Maintain regular and substantial contact with individuals receiving services
24. Perform other job related activities as directed by Manager
